Our October sample form should get your students smiling. This Halloween inspired answer sheet replaces the OMR bubble font with pumpkins. The form consists of thirty multiple choice questions and one short answer area. The answer schema follows the A-E format but feel free to customize the form to your liking using the Word document. You can also download the PDF file and Remark Office OMR template file. Please keep in mind that making changes to the Word document will require you to reset and modify your Remark Office OMR form template file to accommodate your revisions.
